The population of Dereham is expanding rapidly with substantial new areas of housing both under construction and approved. For a considerable period of time the 3 town based practices had closed lists; to increase capacity locally the Southern Norfolk PCT asked Elmham surgery to prepare a proposal for a new surgery at Swanton Morley, a large village 3 mile outside Dereham.
The practice had approval to run this as a Greenfield Nurse-led PMS 3 year pilot, the surgery accommodation is rented form the private sector and is based on the site of a Nursing Home, which also provides Intermediate Care beds. Swanton Morley Nurse-led Surgery opened February 2004 and it is anticipated that 1500-2000 patients will ultimately use the Nurse-Led Surgery.
New GP, Nurse Practitioner and Practice Nurse posts have been required to service this increase in the practice list size and duties are divided between the Swanton Morley Surgery and the main surgery in Elmham. This allows GPs and the Nursing Team to rotate between the Elmham and Swanton Morley Surgeries in order to facilitate team working and ensure streamlined healthcare provision across the practice populations.
It is anticipated that the Nurse-Led PMS Surgery will provide new and innovative ways of working for nurses and provide new opportunities for patients to access healthcare. Nursing skills will be maximized with the Nurse Practitioners and Practice Nurses working at the forefront of their professional boundaries. A Substantial training budget is available to support the professional development of all members of the nursing team.
