Community Car Scheme Medical Form

I am applying to be a Volunteer Driver for my local Community Car Scheme. Part of my role in this scheme is to transport people in my own vehicle for medical related journeys. This could include those who are vulnerable, with disabilities and/or a range of medical conditions, or who are minors. They may need to be driven to their own GP Surgery or to attend hospital appointments.

With a view to protecting the passengers, the public and the car scheme, as well as myself, it is important that I have been certified medically able to undertake such volunteer work. I have therefore been asked to obtain confirmation in the form of a letter from my GP in regard to my health.

I require your confirmation please, that I satisfy the DVLA Group 1 medical standards of fitness to drive, as outlined in the DVLA’s publication “At a Glance to the Current Medical Standards of Fitness to Drive.”
